What do I need to sign up for CDH Balanced Fund?

Very simple:

• A valid identity card (e.g. Passport, Driver's license, National Health Insurance Scheme (NHIS) card.

• A recent passport-size photograph

• A completed application form (available at CDH Savings and Loans Ltd, Phoenix Insurance Company Ltd, Phoenix Life Assurance Company Ltd. ADB, UMB, uniBank and HFC Bank?

• Amount to be invested.
